This is a juried show. We accept handmade crafts, boutique items, and home-based party products. Please send up to three photographs, labeled with your name, showing the type of items you will sell. Please include a photo of your booth set-up to aid in assigning booths. You will be allowed to sell only the type of items shown in the photos. Certain categories may be limited to promote success for everyone.

We are required by law to obtain your sales tax permit number. We send a list of crafters/consultants’ names to the Iowa Department of Revenue and they send temporary permits to us for those who do not have permanent permits. We will give them to you on the day of the show. You must return the temporary permit to the Department of Revenue and pay sales tax within 20 days of the show.
I hereby release all organizers and sponsors of this Craft Boutique including, but not limited to Grace United Methodist Church, from any and all injuries and liabilities of any kind or nature which may arise from my participation in the Craft Boutique.

Please return this form and your check by September 1st. Applications received after this date will be accepted based upon availability. No booth reservation will be held without payment. Notification of acceptance will be made via email. Please include a self-addressed, stamped envelope for return of photos and space rental fee in the event we do not have space available for your business.
